What do I need to bring with me in order to see the doctor?

* Current insurance card and picture ID

* Means of payment if you do not have group health insurance. We take all major credit cards.

* Referral number from primary care physician if you are a member of an HMO or POS plan

* Previous x-rays, MRIs, and other test results

* Any questions you wish answered by the doctor during your visit. We suggest you write them down and bring that list with you.

   
 

What do I need to do if my injury is work related?

* Notify your employer of the injury immediately. Fill out forms as directed by your employer.

* Make sure your employer has reported your injury to the Workman's Compensation Carrier.

* When calling for an appointment at our office, tell the scheduler that this is an "on-the-job injury."

* Have all information regarding your employer, including address, phone number and supervisor's name available at time of scheduling.

* If seen in the emergency room prior to your office visit, bring all x-rays from that hospital with you when you visit our office.

   
 

How do I obtain a work release or school release?

Notify the doctor or assistant at the time of your examination.

   
 

How do I get a prescription refilled?

If this office issued the original prescription, call your pharmacist for a refill. DO NOT call our office. Your pharmacist will contact us directly for authorization.

   
 

What do I do if I have a problem with my prescription medicine?

Stop taking the medication immediately. Contact our office and speak to the doctor assistant for further instructions.

What are your hours of operation?

Office hours are 8:30 a.m. to 5:00 p.m., Monday through Friday. Doctors' hours vary daily due to scheduling of surgeries.

   
 

Do your doctors specialize in spine surgery?

Our doctors are general orthopedic surgeons. If you have had back surgery, you need to see the doctor who performed the surgery or find a spine specialist.
